The Exam

Choose the appropriate forms below for the practitioner(s) you will be seeing. 

Simply print them off, fill them out, and bring them to your first appointment. 

The forms are extensive and can take about an hour to complete. Do your very best filling them out. The more information we can gather about you and your condition the better we can ensure appropriate and effective care.

We use a variety of evaluation and treatment techniques specific to each case. We will listen to your story and ask a number of questions relating to your condition.

The physical exam may include looking at your posture, skin tone, eyes, ears, and tongue. If necessary, we may gently palpate or touch your joints and muscles to check for heat, movement, and tension. 

Dr. Brad may use muscle reflex techniques like applied kinesiology to help him in the evaluation process. This consists of gently challenging certain muscles to see how they reflexively respond. Treatment may include a number of therapies including specific chiropractic adjustments, muscle therapies, nutritional recommendations, therapeutic exercises, or referral to a specialist.

 

WHAT YOU NEED TO BRING TO YOUR FIRST DR. BRAD VISIT

  • Your patient forms

  • All medications or vitamins you are currently taking—bring the product in the original bottle, not just a list

  • Any lab results from the past three months

We do have a 48-hour cancelation policy. Please arrive a few minutes before your appointment. If you have any questions or concerns about your first appointment, don't hesitate to give us a call. Keep in mind we can always offer a free phone consultation to new patients.

 

Fees & Payment for Service

We believe you and your doctor should make decisions about your healthcare—not an insurance company.

Our goal is to provide the most appropriate care for your specific case. We base our treatment recommendations completely on appropriateness for you and not on insurance reimbursement. For this reason, we do not submit insurance claims.

Our fees are based on the time scheduled for each appointment rather than by each service performed. This allows us to offer the best possible care for your specific case. This also allows you to choose the amount of time you would like to spend directly with your Specialist.

You will be aware of your appointment fee regardless of whether the care provided includes chiropractic adjustments, muscle testing, nutrition evaluation, muscle release techniques, exercise therapy, lifestyle education, or other services.

Payment is expected at the time of service. We accept cash, check, and all major credit cards.
